Onsite Attendance Booking Form

Onsite Attendance Booking Form: Mon 23rd Aug to Fri 27th August 2021

Please complete the booking form details to register your child for Onsite Attendance during Remote Learning.  All students must be pre-registered to allow the school time to coordinate staffing. Late bookings will be subject to availability.

Onsite Attendance is only STRICTLY available where Authorised Workers are able to provide AN AUTHORISED WORK PERMIT* and that they cannot work from home OR are a vulnerable student as per the guidelines and in consultation with the school. The school also reserves the right to also permit Onsite Attendance in negotiation with families as long as they meet the guidelines as specified by the Chief Health Officer.

Schools will be closed during this time but will stay open for students in the following categories:

(1) Students of parents/guardians considered Authorised Workers who cannot work from home.

  • Where there are two parents/guardians, both must have an Authorised Worker Permit, and be working outside the home
  • For single parents/guardians, they must must have an Authorised Worker Permit and must be working outside the home

* An Authorised Worker Permit is not required for people employed in law enforcement, emergency services or health care workers who carry employer-issued photographic identification to be allow permission to be away from home.  At a school level we request an official document detailing your shifts for the week.  At LRH for example Unit Managers are still using the actual Authorised Work Permit to identify the shifts a nurse is working that week, another example being an Aged Care Facility that has an official workplace roster that can be sighted by school leadership.

(2) Vulnerable students including:

  • children in out-of-home care
  • children deemed by Child Protection and/or Family Services to be at risk of harm
  • children identified by the school as vulnerable (including via referral from a family violence agency, homelessness or youth justice service or mental health or health service)

All students must be signed IN and OUT via the QR Code displayed in the school office window.
